COVID-19 Test Information

EXEMPLAR CARE will be providing on-site COVID-19 testing at the Iowa Events Center on Friday, September 10, 2021 on behalf of the USA/Canada Lions Leadership Forum.  Testing will be available between 12:30 pm and 5:30 pm in Room 333.

Note:  Some off-shore areas may have difficulty accessing the system steps described below.  If so, please contact Jaci Robson (Jacqueline@exemplar.care) or call 515-650-4370 for 24/7 assistance.

Please complete the steps below to register for your COVID-19 test:

  1. REGISTRATION: Navigate to our online website: Iowa Events Center Lions Leadership Forum COVID-19 Testing  (https://www.exemplar.care/usa-canada-lions-leadership-forum)
  2. LOCATION – Select Testing will take place on-site at the Iowa Events Center during the USA/Canada Lions Leadership Forum. The booking link will state WEST DES MOINES as results are linked to this clinic
  3. TEST TYPE – Select test type required, the following tests are being offered for testing on-site at Iowa Events Center
    • PCR TESTING (NASAL OR NASOPHARYNGEAL)
      • Cost: $90
      • Results Available: Within 24 hours for travelers. Normal turnaround time is from 24-48 hours.
      • Considered the gold standard, and accepted for international travel
      • FDA Compliance: This test has received Emergency Use Authorization (EUA) from the FDA
      • Polymerase Chain Reaction (PCR) tests are molecular tests used as a diagnostic test to show if a person has an active coronavirus infection and should take steps to quarantine or isolate themselves from others. This test is administered as an anterior nasal swab OR a nasopharyngeal swab
    • RAPID ANTIGEN TESTING
      • Cost: $65
      • Results Available: 1 hour or less
      • FDA Compliance: This test has received Emergency Use Authorization (EUA) from the FDA
      • COVID-19 Rapid Testing is now available. Antigen COVID-19 tests detect proteins from the virus. Antigen tests are very specific for the virus but are not as sensitive as molecular tests. This means that a positive result is highly accurate, but a negative result does not rule out infection. This test is administered as a nasal swab. We are using the BD Veritor system antigen testing.
  4. DATE AND TIME – These links will only allow you to select an appointment on Friday September 10th between 12:30pm – 5:30pm. Once selected, click NEXT STEP twice
  5. ACCOUNT SETUP – Enter the requested information (name, date of birth, address, phone number) to setup your account in our secure portal.
  6. PATIENT – Indicate appointment patient by selecting IT IS FOR MYSELF
  7. REQUIRED FORMS – Complete the Exemplar Care Consent Form and HIPPA Information Form.
  8. Select PLACE YOUR ORDER
